Application and Fees for Certificate (Price for NHI card holder only)
If there is any adjustment to the fees, the newly announced fees shall prevail.
Required certificate for application:
1.Registration certificate and health insurance card
2.Original of national identification card (use Household Registry for children) for the patient and agent or passport (foreigners)
